Reordering printed caresheets for pharmacists
If you are a pharmacist using the print version of PharmacyLive, this page contains the information you need to reorder more caresheets.
UBM Medica (NZ) Ltd (formally CMPMedica (NZ) Ltd) is happy to resupply PharmacyLive printed caresheets free of charge to subscribers to the service where you have run out of these. These will be sent out in packs of 25 leaflets, to fit the container capacity.
If you still have the reorder form originally supplied with the PharmacyLive print kit and container, you can photocopy this and fax the completed form to (09) 489 6240.
If you do not have this, you can find the reorder form (as a pdf file).
You may also email your reorder request to reorder@pharmacylive.co.nz – please ensure we have all of the details requested on the actual form.
